Frontline Care Management Team

This team implements day-to-day operational procedures, working directly with the people we support, their families and our care staff to ensure high-quality, person-centred care and service delivery.

They help ensure legal compliance is maintained and best practice is followed every day. The team supports our dispersed workforce, identifies training needs, works in partnership with district nurses, physiotherapists, occupational therapists, hospital discharge coordinators and family carers, and plays an important role in shaping the direction of the organisation.

They are also responsible for managing change, covering out-of-hours requirements and overseeing the scheduling of care provision.

Call Monitoring Team

Our Call Monitoring Team oversees the delivery of care each day, ensuring that all visits are attended within acceptable timescales.

They support front-line care staff throughout their shifts, providing guidance where needed and escalating any concerns to Senior Field Supervisors. At the end of each shift, they complete a handover for the Care Management Team, outlining the performance and any key updates from that shift.

The team also acts as a front-line point of communication for service users regarding their care provision and care staff.

During their allocated shifts, the Call Monitoring Team is responsible for monitoring and supporting the smooth delivery of all scheduled care services.

About: Experience and Qualifications: I joined Care in Hand in January 2023 as a community carer until October 2024. Got a promotion and now l am working as a Call Monitor. 

In terms of academics, I am working towards completing my level 2QCF

 

Fun Fact/Favourite Pastime: I enjoy spending time with family and going out . My aim is to donate more towards charity and travel the world.
