Supported Living Services

As a supported living service provider, we play a vital role in supporting individuals with disabilities, Learning difficulties and Mental Health needs to live fulfilling and independent lives at home and within the community

Key Aspects:

As highly trained social care professionals, we go over and beyond to ensure that a partnership approach is considered at all times so that individuals can achieve their personal wellbeing outcomes.

Key aspects:

  • Person-Centred Approach: prioritise a person-centred approach, meaning that services and support are tailored to meet the unique needs, preferences, and goals of each individual. This involves collaborating closely with individuals, their families, and support networks to develop and implement individualized support plans.
  • Housing: options that meet the specific needs of the individuals – signpost & support
  • Comprehensive Support Services: Offer a range of support services aimed at promoting independence and well-being. This may include assistance with daily living skills, healthcare coordination, social inclusion activities, vocational training, and employment support.
  • Qualified Staff: Employ qualified and compassionate staff members who are trained to work effectively with individuals with diverse abilities. Staff have the skills and knowledge necessary to provide person-centred care, promote autonomy, and respond to the unique needs and preferences of each individual.
  • Advocacy and Rights Protection: Advocate for the rights and interests of the individuals we support, ensuring that they have access to necessary resources, services, and opportunities. This may involve advocating for policy changes, addressing discrimination, and promoting inclusive practices within the community.
  • Collaboration and Partnerships: Establish collaborative relationships with community organizations, healthcare providers, educational institutions, employers, and other stakeholders to enhance support services and create opportunities for community integration and inclusion.
  • Continuous Improvement: Regularly evaluate and assess the effectiveness of our support services and seek feedback from individuals, families, and stakeholders. Use this feedback to identify areas for improvement and make adjustments to enhance the quality and impact of our services.

By fulfilling these responsibilities, we can make a meaningful difference in the lives of the individuals we support, helping them to achieve their goals, maximize their independence, and participate fully in their communities. For our Easy Read version of this page please click here
