Front Line Care Management Team
What do they do?
This team implement day to day operational procedures, working directly with people who need care and carers, individual care and service delivery, they ensure legal compliance is implemented and best practice is achieved daily. They support a dispersed workforce, identify training needs partnership working with district nurses, physiotherapists, OT, Hospital discharge coordinators and family carers. The team is result–focused contributing to informing the direction of the organisation and manage change, and cover out of hours requirements. This team is responsible for all scheduling of care provision. Meet some of the team below.

Call Monitoring Team
What do they do?
Our Call Monitoring Team monitor our provision of service each day. Their role consists of ensuring that all visits are attended within acceptable timescales. They support the front-line care staff during their shifts. Provide guidance where necessary and escalate any concerns to Senior Field Supervisors each shift. They are responsible for completing a handover at the end of each shift to the Care Management Team in regards to the performance of that shift. They are the front-line communicators with our Service users in regards to their care provision and carers.
Our Call Monitoring team are responsible for covering all provision of service during their allocated shifts.
