Front Line Care Management Team

What do they do?

This team implement day to day operational procedures, working directly with people who need care and carers, individual care and service delivery, they ensure legal compliance is implemented and best practice is achieved daily. They support a dispersed workforce, identify training needs partnership working with district nurses, physiotherapists, OT, Hospital discharge coordinators and family carers. The team is resultfocused contributing to informing the direction of the organisation and manage change, and cover out of hours requirements. This team is responsible for all scheduling of care provision.  Meet some of the team below.

Call Monitoring Team

What do they do?

Our Call Monitoring Team monitor our provision of service each day. Their role consists of ensuring that all visits are attended within acceptable timescales. They support the front-line care staff during their shifts. Provide guidance where necessary and escalate any concerns to Senior Field Supervisors each shift. They are responsible for completing a handover at the end of each shift to the Care Management Team in regards to the performance of that shift. They are the front-line communicators with our Service users in regards to their care provision and carers.  

Our Call Monitoring team are responsible for covering all provision of service during their allocated shifts.

About: Experience and Qualifications: I started care in hand Jan 2016 as a casual carer, I have worked my way up in the company and I am now currently in the senior field supervisor position overseeing the Milford haven and Haverfordwest teams. I have completed my level 3 QCF and currently working towards my level 5. 

Fun Fact/Favourite Pastime: I enjoy travelling and I have visited many countries. My aim in life is to travel the world and experience all different cultures and their way of living… reality check I need to start doing the lottery.

About me 

I have worked in care for 12 years, starting with children with learning difficulties and challenging behaviour, I then worked in a psychiatric hospital before moving to Wales in 2016, I then worked in a supported living setting in Carmarthen before starting with Care in Hand in 2018, I started as a call monitor, then a field supervisor nad now working as a senior field supervisor for the Wooden, Narberth and Carmarthenshire teams. I am working towards my level 4 QCF.

What I enjoy

I enjoy spending time with my children and dog, going to the beach and chilling.

Samual Merayo

About: I started working for Care in Hand in 2017 as a community carer. I then became a Field Supervisor and I am now the Senior in the North. I have completed my level 3 QCF in health and social care and will soon start working towards my level 4 and 5. 

Fun fact: I was born in Spain where I lived until I came to Wales. All my family members are still in Spain and I go to visit as much as I can. I like playing video games and spending time with my cats when I am not visiting the beautiful Pembrokeshire coast towns.

About:

I started with care in hand in 2021, I have 12 years’ experience working with ALN and have 6 years’ experience within SLS.  I have completed my level 3 in health and social care and will be starting my level 4 in the near future.

 

Fun Fact/Favourite Pastime: I am passionate about cold water swimming and swim all year round on our beautiful beaches here in Pembrokeshire.

About: Experience and Qualifications: I joined Care in Hand in January 2023 as a community carer until October 2024. Got a promotion and now l am working as a Call Monitor. 

In terms of academics, I am working towards completing my level 2QCF
